  1. Enter your juror "Badge Number" (The "Badge Number" is located on the Juror Badge of your summons).
  2. Enter your date of birth. (Use the MM/DD/YYYY format)
  3. Enter your five digit zip code. DO NOT enter a nine digit zip code.
